Please find attached advanced version of SOLI 18/12/2018 The Swedish Transport Agency has turned to French CAA to fill out the SOLI form. Could you please clarify the situation with French CCA. The french Medical Office received my medical file (renewal visits for 2016 - 2017 - 2018) from you and transferred it to Sweden with a SOLI "From France to Sweden". The Sweden Authority noted that the ITEM Nr 8, Page 2 of the SOLI was not properly completed and asked us to fill it out correctly. I have attached a new SOLI, could you please fill it out and send it back to me ? Would it be possible for you to provide me with this document as soon as possible because I should start my training shortly but my licence is not yet issued ? Thank you so much for your cooperation.
